About The Position

A high-acuity, Level I trauma acute care hospital in Nashville, Tennessee is seeking an accomplished Director of Case Management to provide strategic and operational leadership for hospital-wide case management services. This role oversees all aspects of care coordination, utilization management, discharge planning, and interdisciplinary collaboration across a large inpatient environment. The Director will lead performance improvement initiatives focused on length of stay optimization, throughput, regulatory compliance, and patient-centered transitions of care. This opportunity is best suited for a seasoned case management leader with demonstrated success in complex acute care settings and the ability to partner effectively with executive, physician, and clinical stakeholders.

Requirements

  • Bachelor’s degree in Nursing, Social Work, or a related healthcare field
  • Active licensure as one of the following: Registered Nurse (RN) Licensed Clinical Social Worker (LCSW) Licensed Master Social Worker (LMSW)
  • Minimum of 3 years of recent acute care hospital case management experience
  • Minimum of 2 years of case management leadership experience
  • Demonstrated success managing hospital-wide or multi-unit case management operations

Nice To Haves

  • Master’s degree in Nursing, Health Administration, Business Administration, or related field
  • Experience leading case management in a Level I trauma or high-volume acute care hospital
  • Strong background in data analysis, process improvement, and length-of-stay reduction
  • Proven ability to lead multidisciplinary teams and influence across departments

Responsibilities

  • Provide executive leadership for hospital-wide case management operations.
  • Direct and evaluate departmental structure, staffing models, onboarding, competencies, and productivity.
  • Ensure compliance with regulatory, accreditation, and utilization management standards.
  • Oversee care coordination and discharge planning processes to support safe, timely patient transitions.
  • Partner with finance and executive leadership to achieve operational, quality, and fiscal goals.
  • Analyze performance data to identify trends, risks, and opportunities for improvement.
  • Lead and sustain performance improvement initiatives targeting length of stay and capacity management.
  • Build collaborative relationships with physicians, nursing leadership, compliance, legal, and ancillary services.
  • Serve as a subject matter expert in acute care case management and care coordination best practices.
  • Support change management initiatives and lead teams through operational transformation.
