About The Position

This role is responsible for driving outcomes, procedures, and processes of the system-wide Case Management program in the adult hospitals and reports to the Executive Director of Care Coordination. This leader partners with the Executive Director of Care Coordination to provide strategic and operational oversight to ensure high-quality, efficient, and patient-centered care coordination throughout the inpatient setting. This role is also responsible for direct oversight and mentoring of the Managers of Inpatient Case Management and driving accountability as it pertains to leadership supervision, operational excellence, throughput initiatives, length of stay, readmission prevention, and capacity management. The role partners closely with Social Services leadership developing and implementing team-based models of care resulting in positive patient outcomes and operational success. This role ensures patients and families have seamless access to care across the care continuum and will serve as a strategic partner to hospital and health system leadership.

Requirements

  • Experience in case management program oversight.
  • Experience in strategic and operational oversight of care coordination.
  • Experience in mentoring managers.
  • Experience in driving accountability for leadership supervision, operational excellence, throughput initiatives, length of stay, readmission prevention, and capacity management.
  • Experience partnering with Social Services leadership.
  • Experience in developing and implementing team-based models of care.
  • Experience ensuring seamless access to care across the care continuum.
