About The Position

This position involves providing direct support to individuals, assisting them with daily activities, offering emotional support, and facilitating their integration into social activities and the community. The role requires implementing care plans, documenting observations, and maintaining a safe environment. Requirements

• Ability to lift and perform heavy physical tasks as required
• Experience or knowledge in providing care for individuals with dementia or cognitive impairments
• Valid driver's license and ability to drive individuals to appointments or community outings
• Familiarity with assisted living facilities, nursing homes, or residential care settings
• Knowledge of medication administration procedures and protocols
• Strong interpersonal skills and the ability to work effectively as part of a team
• Understanding of direct support systems and practices

Nice To Haves

• Background in social work or related field is a plus

Responsibilities

• Assist individuals with daily activities such as personal hygiene, meal preparation, and medication administration
• Provide emotional support and companionship to individuals
• Assist with mobility and transfers, including heavy lifting when necessary
• Implement care plans and follow established protocols for individuals with specific medical conditions or disabilities
• Support individuals in participating in social activities and community integration
• Document observations and report any changes in behavior or health status to supervisors
• Maintain a clean and safe environment for individuals
