Direct Support Provider
About The Position
As a Direct Support Provider, you will deliver person-centered services that support individuals with special needs in building independence at home and in the community. This role exists to help members work toward goals outlined in their service plan while maintaining safety, dignity, and respectful support. Your shift begins by checking the member’s current goals, recent notes, and any family updates so support stays consistent. During evenings, you may help guide routines and plan structured activities that align with the service plan. Shifts may include scheduled outings where goals can be practiced in real-world settings. Throughout nights, evenings, and weekends, you’ll keep communication clear, document services promptly, and follow safety expectations so families and team members have accurate, reliable updates from visit to visit.
Requirements
- Motivated to make a positive impact
- Able to advocate for members and promote self-esteem
- Valid Arizona Level 1 Fingerprint Clearance (or ability to obtain)
- Valid driver’s license and reliable transportation
- CPR/First Aid certification (or ability to obtain)
- Article 9 and Introduction to Developmental Disabilities training (or ability to attend)
- Flexible availability including evenings, nights, and weekends
- Strong communication skills
- Strong documentation skills
- Strong safety-compliance skills
