About The Position

St. John's Community Services (SJCS) is seeking a Day Support Specialist to join their team. This role involves providing direct support to individuals with disabilities, helping them develop and maintain skills to enhance their quality of life in the community. The specialist will assist with personal care and daily routines in community training sites, acting as an active and contributing member of a dynamic and collaborative team. Requirements

  • Ability to gain working knowledge of St. John’s Community Services policies and procedures.
  • Ability to act as a representative of St. John’s Community Services to the public.
  • Ability to effectively communicate and coordinate with other staff members and individuals from diverse backgrounds.
  • Knowledge of acceptable food preparation and safety practices and procedures.
  • Ability to model and teach relevant social and behavioral skills.
  • Ability to operate relevant computer systems, including hardware and software, and office machines.
  • Obtain a valid health certificate to meet SJCS or current funding authority requirements as required.
  • Meet SJCS and the funding authority requirement of criminal background and record checks.
  • Valid state-issued driver's license with current driver's record in good standing.
  • May lift, position and/or transfer individuals with disabilities.

Nice To Haves

  • High school diploma or G.E.D. preferred.
  • Two or more years’ experience working with people with disabilities preferred.
  • Experience in community-based day programs a plus.

Responsibilities

  • Carrying out the mission and core values of the organization through relationship building activities internally and externally, as well as skills training acquired because of the implementation of the Individual Service Plan (ISP).
  • Facilitating individual’s active participation in community-based activities, including contracted services and volunteer activities.
  • Completing daily and routine data collection and documentation as stipulated in the Individual Service Plan (ISP) including behavioral support and other specialized plans.
  • Observing, responding, and reporting/documenting individual’s physical, social, behavioral and personal needs by recognizing any changes in individuals that may require additional follow-up or attention.
  • Monitoring and reporting any unusual events in accordance with SJCS policy and state regulations.
  • Assisting in the development of activity calendars to ensure that the activities conducted meet the needs of the individuals supported as indicated in ISPs.
  • Providing safe transportation to and from scheduled community activities, appointments, and/or community sites.
  • Providing routine feedback regarding fleet transportation services as needed and requested.
  • Providing adequate supervision to individuals to facilitate their health and safety including administering medication that is timely and in the correct dosage.
  • Attending and participating in all relevant agency events, workshops, training and meetings.
  • Assisting with medical appointments.
  • Performing other job-related duties as assigned.
