Requirements

- Valid driver's license
- High school diploma or GED
- Ability to complete required training and certifications
Responsibilities
- Support individuals in achieving personal and community-based goals
- Assist with daily routines and life skills development
- Promote safe, healthy, and independent living
- Provide transportation to appointments, activities, and community outings
- Monitor health, safety, and well-being
- Document services and progress accurately
- Work collaboratively with families and interdisciplinary teams
