About The Position

This position involves providing direct support to consumers, ensuring their health, safety, and comfort in their homes. The role requires adherence to Individual Service Plans (ISPs), performing personal care, household tasks, and accompanying consumers to appointments. Flexibility in scheduling, including evenings, weekends, and potential overtime, is necessary. Maintaining client confidentiality is paramount.

Requirements

• High School Diploma or Equivalent
• Valid Ohio Driver's License – with less than 6 points
• Reliable transportation with proof of insurance
• CPR/First Aid certification
• Working Cellular Phone – to clock in and out for billing purposes
• Must be able to lift, carry, push or pull a minimum of 50 pounds.
• Frequently requires sitting, standing, walking, bending, squatting, kneeling and reaching.
• May be required to be on feet hours at a time.

Nice To Haves

• One to four years of college in a related field is preferred

Responsibilities

• Participate in the consumer's Individual Service Plan (ISP) meetings when requested.
• Perform tasks and duties according to the ISP, prioritizing and organizing tasks to achieve ISP goals.
• Maintain a clean and safe environment, being sensitive to the consumer's and family's needs.
• Recognize and report changes in the consumer's condition and behavior, as well as safety and sanitation hazards.
• Provide basic personal care and grooming, including bathing, hair care, and assistance with clothing.
• Perform household services essential to the consumer's health and comfort, such as changing linens or rearranging furniture.
• Perform homemaking tasks such as cooking, feeding, cleaning, and shopping.
• Accompany consumers to medical appointments, walks/outings, and other appointments.
• Maintain client confidentiality as required by County DD standards and HIPAA regulations.
