About The Position

Take Another Chance is seeking a Direct Support Professional (DSP) to join our team. DSPs provide hands-on support to individuals with developmental disabilities in a 24-hour awake care residential setting, helping them live safe, independent, and meaningful lives. We are looking for dependable, compassionate professionals who align with our values: Compassion, Opportunity, Dignity, and Evolvement (C.O.D.E.). This position requires reliability, attention to detail, and a strong commitment to continuity of care. This is a part-time position with scheduling based on client needs.

Requirements

  • Must be at least 18 years old
  • High school diploma or GED required
  • Valid state driver’s license
  • Reliable daily access to a personal vehicle
  • Active auto insurance meeting state minimum requirements
  • Ability to pass both a criminal and a driving history background check
  • Current AHA Heartsaver First Aid/CPR/AED certification
  • Current Category I Medication Administration Certification with the Ohio DODD
  • 1 year prior experience supporting individuals with developmental disabilities
  • Ability to lift at least 50 lbs and perform physical tasks (bend, reach, stand for extended periods)
  • Ability to work assigned shifts consistently to support continuity of care
  • Basic computer literacy, including the ability to learn and use systems for documentation, communication, and training
  • Strong written and verbal communication skills to ensure accurate, timely documentation and clear communication across shifts
  • Professional interpersonal skills to support individuals served and maintain positive working relationships with team members
  • Ability to work independently and collaboratively within a team to support continuity of care and consistent service delivery

Nice To Haves

  • Proficiency with electronic documentation systems and Microsoft Office Suite
  • Current Category II or Category III Medication Administration Certification with the Ohio DODD
  • 1+ years prior experience supporting individuals with developmental disabilities

Responsibilities

  • Assist with activities of daily living including bathing, grooming, toileting, and feeding
  • Support individuals with mobility and physical assistance as needed
  • Prepare meals and assist with nutritional needs
  • Administer medications in accordance with Ohio DODD standards and agency policy
  • Monitor and report changes in behavior, health, or safety concerns
  • Follow all ISP protocols and safety plans
  • Maintain a clean, safe, and organized living environment
  • Provide transportation to appointments, activities, and community outings
  • Support skill-building and independence in daily routines
  • Accurately complete daily documentation and reports
  • Follow all company policies, ISP requirements, and state regulations
  • Participate in required trainings and maintain certifications
  • Build positive, respectful relationships with individuals served
  • Encourage participation in activities and community engagement
  • Provide emotional support while promoting independence
