About The Position

Under the supervision of the Edgewood Program Manager, provide supervision, support and training to individuals with intellectual disabilities who live in the Edgewood Group Home.

Requirements

  • High School diploma or G.E.D.
  • Valid driver’s license and proof of insurance.
  • Must maintain reliable transportation and drive personal vehicle to all scheduled shifts.
  • Must be willing and able to work flexible hours to include holidays, evenings and weekends.
  • Attend required MR/DD training and all annual updates necessary.
  • Criminal background check and drug test to meet agency/SCL standards.
  • Must maintain telephone access and provide current phone number to supervisor to ensure timely communication.
  • Capable of lifting 50 lbs.
  • Provides direct services to clients.
  • Will document work done daily and monthly.
  • Will perform tasks directly related to training received.

Responsibilities

  • Serve as a teacher/mentor, providing person centered-training, instruction, and assistance to support individuals to achieve desired outcomes and develop independent living skills.
  • Provide flexible, individualized supports and assistance with: cooking, meal planning, shopping, budgeting, bill paying, banking, laundry, self advocacy, solving day-to-day problems, developing friendships, interacting with neighbors, household tasks and maintenance, recreational planning, accessing community resources, personal care, emotional support, medication administration, and transportation.
  • Assist individuals to develop and maintain positive relationships (i.e. family, friends, neighbors, community members, clubs, civic organizations etc.)
  • Assist individual to address Residential objectives (as documented on the Individual Support Plan) and document accordingly.
  • Assist individuals with personal care needs (i.e. bathing, dressing, toileting, dental hygiene, shaving, eating etc.)
  • Assist with medical appointments, ordering/refilling medications, and proper medication administration.
  • Follow all medical, dietary, and documentation requirements.
  • Make introductions and serve as a bridge between individuals and community members and facilitate meaningful community participation/involvement.
  • Promote, teach and model personal growth and self-esteem (i.e. personal grooming, table manners, social skills, conflict resolution, etc.)
  • Conduct oneself morally, and ethically, serving as a positive role-model at all times.
