About The Position

UCP Heartland is seeking an enthusiastic and compassionate PRN Direct Support Professional (DSP) to support adults with developmental and physical disabilities in our NORTH COUNTY ISLs. As a PRN DSP, you’ll play a vital role in fostering independence, enhancing quality of life, and ensuring safety and dignity for the people we serve—both in their homes and the community. Requirements

  • Outgoing, fun and positive personality
  • Strong interpersonal and relational skills—professionalism with individuals served, coworkers, families, and external contacts
  • Highly reliable, organized, and creative with a collaborative mindset
  • Comfortable working independently and within a team
  • Willingness to learn and utilize technology platforms including: Relias, QuickMAR, Set-Works, Microsoft 365, and ADP
  • Ability to follow through on responsibilities and maintain confidentiality
  • Valid driver’s license and clean driving record
  • Minimum requirement for 8 shifts in a month and that will include at least 2 weekend shifts.
  • Must maintain or be willing to obtain the following (training support provided): Level 1 Medication Aide (L1MA), First Aid/CPR, Crisis Prevention & Intervention (CPI)
  • Must adhere to PRN contract and maintain active training status
  • Requires reliable transportation, valid driver's license with clean driving record!
  • Requires 5-7 days of mandatory training at our corporate office Monday through Friday 8:45 AM - 4:15 PM

Nice To Haves

  • Experience supporting individuals with developmental or physical disabilities
  • Previous ISL or direct care experience

Responsibilities

  • Assist individuals with Activities of Daily Living (ADLs) including bathing, grooming, dressing, toileting, and meal preparation
  • Safely perform lifting and transfers, including use of ceiling lifts and wheelchair repositioning
  • Administer medications per physician orders and agency policies (L1MA required or willing to obtain)
  • Provide adaptive transportation to community outings, appointments, or events
  • Implement and support ISP (Individual Support Plan) goals and activities
  • Maintain shift documentation using Set-Works and complete reports as required
  • Communicate effectively with families, guardians, coworkers, and agency staff
  • Maintain a safe, clean, and positive environment, including light housekeeping duties
  • Participate in team meetings, calls, and ongoing collaboration with supervisors
