About The Position

Accord is seeking a Direct Support Professional to join our Family Support Services team (245D Basic Services). Accord is a nonprofit dedicated to making it possible for people living with disabilities to achieve their personal and career goals. We utilize a person-centered approach to provide services that span a wide array of supports to empower, uplift, and transform lives to help individuals live their greatest lives. Accord has been recognized by the Council on Leadership and Quality for Excellence, with Distinction, in person-centered care. As a Direct Support Professional, you will provide non-medical care, including supervision and assistance provided in the home of the person or in the community to help the person receiving services achieve increased independence, productivity, and inclusion in the community. The DSP will develop real, meaningful relationships with the person supported and their support team. Staff will follow through on all duties as assigned by the primary contact, including outings into the community, assistance with ADLs, and overall supervision.

Requirements

  • Minimum of a high school degree or GED (or relevant/equal education)
  • Reliable transportation and valid driver’s license
  • Minnesota DHS background study clearance
  • Have a Wi-Fi-enabled device for work related purposes
  • Basic computer skills
  • 18 years of age or older
  • Ability to read, write, and speak the English language

Nice To Haves

  • Previous experience in working with persons with disabilities
  • Strong communication and interpersonal skills
  • Knowledge of person-centered care approach
  • Ability to maintain confidentiality with attention to privacy standards
  • Ability to serve as ambassador of Accord in the community
  • Female DSP preferred

Responsibilities

  • Assist individuals with daily living activities such as cooking/meal preparation, showering and personal cares
  • Support the person in becoming more independent and involved in their community through frequent activity outings
  • Assist with walks and supervision while at home and in the community
  • Complete detailed, accurate documentation as assigned
  • Comply with Electronic Visit Verification documentation
  • Maintain a clean and safe living environment
  • Implement individualized care plans and ensure individual’s rights are respected
  • Provide companionship and engage individuals in recreational activities
  • Any other duties as assigned by the individual’s primary contact within the scope of service.
