About The Position

Provide individualized, in-home and community-based support to individuals with disabilities. This role focuses on promoting independence, developing life skills, and supporting individuals in achieving personal goals while remaining in their home environment.

Requirements

  • Strong interpersonal and communication skills
  • Ability to work independently in home and community settings
  • Empathy and ability to support individuals with diverse needs
  • Strong organizational and documentation skills
  • Ability to collaborate effectively with team members and families
  • Basic computer skills (word processing, spreadsheets, and data entry)
  • Ability to lift and carry up to 50 pounds
  • Frequent bending, stooping, and assisting individuals with mobility
  • Ability to travel between client homes and community locations
  • Valid driver’s license, reliable transportation, and proof of insurance required
  • CPR and First Aid certification
  • Preventing Disease Transmission (PDT)
  • Certified Restraint Training (CRT)
  • Medication Administration (Core A & B)
  • Completion of required trainings
  • Experience working with individuals with disabilities is required

Nice To Haves

  • High School Diploma or equivalent preferred.

Responsibilities

  • Assess individual needs and assist in developing and implementing individualized service plans
  • Provide training in daily living skills, including personal care, communication, and social development
  • Support individuals in accessing community resources and participating in community activities
  • Assist with household tasks, organization, and maintaining a safe home environment
  • Escort individuals to medical appointments and community activities as needed
  • Support individuals with correspondence, budgeting, and bill paying when necessary
  • Maintain accurate documentation of services, progress, and outcomes in compliance with regulations
  • Develop materials and maintain supplies to support individual goals
  • Act as a resource to families, caregivers, and other team members
  • Promote independence, self-advocacy, and personal growth
  • Maintain positive relationships with individuals, families, and community partners
  • Remain flexible to meet the needs of individuals served

Benefits

  • $350 Sign-on Bonus eligible position. Must be scheduled for a minimum of 20 hours/week to be eligible for the sign-on bonus.
