About The Position

The Direct Support Professional III- LPN is a Licensed Practical Nurse who provides direct medical support and education to individuals in assigned programs. This role advocates for all medical needs of assigned program participants, focusing on optimal health. Hours are assigned by a supervisor, with on-call responsibilities. With advanced qualifications or significant experience, this role will provide specialized or high-level support, while maintaining quality, service excellence, and ethical standards.

Requirements

High school diploma, GED, or HiSET required.
Licensed practical nurse required (LPN)
CPR/First Aid and MAP certification required (paid training provided if needed).

1-3 years of related experience preferred.

Responsibilities

Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant clinical and residential staff
Accompany and assist individuals with medical appointments in accordance with DDS guidelines
Document all nursing interventions, health updates, and medication changes in Electronic Medical Records (EMR) and communication logs
Monitor and maintain current medication and treatment orders, report side effects or adverse reactions as appropriate
Review and support implementation of Individual Service Plans (ISPs), including self-medication, behavior plans, hygiene, and therapy programs
Train residential staff on medication administration and health protocols
Ensure up-to-date and compliant health records in line with DDS standards
Communicate medical recommendations clearly to all program staff involved in the individual's care
Maintain safety and monitor for health-related risks within the program
Participate in staff meetings, mandatory trainings, and clinical supervision with the Health Care Director or program RN
Perform medical interventions and additional responsibilities as assigned
