About The Position

Direct Support Professional III provides an array of services which are necessary to service people and their families with needs related to disabilities, mental health or aging to live in community settings and provide companionship services of fellowship, care and protection. Supports are individually planned and tailored to meet the specific needs of the people we serve. This may include working at multiple homes, locations, and departments.

Requirements

  • Must be 18 years of age or older
  • Possess a valid driver’s license
  • Valid insurance with liability or specific plan for transportation, if required.
  • Minimum of three (3) years’ experience working with: Individuals with Mental Health diagnoses
  • Minimum of three (3) years’ experience working with: Individuals exhibiting challenging behaviors
  • Minimum of three (3) years’ experience working with: Individuals with Individual and Developmental Disabilities (IDD)
  • The candidate should demonstrate strong interpersonal skills
  • The candidate should demonstrate the ability to implement supportive strategies
  • The candidate should demonstrate a commitment to promoting independence and quality of life for the individuals served.
  • Requires a criminal background check.
  • Requires the final candidate to successfully pass an E-Verify check.

Responsibilities

  • Implements interventions to persons in crisis; under the supervision of the behavior specialist and lead LCSW/LPC, for In-home and Out-home Crisis Support Services
  • Maintain hyper-vigilance during shifts to provide redirection and other behavioral interventions to individuals in crisis
  • Demonstrate and perform as an effective adult model for people we support
  • Implement ISP goals
  • Keep accurate daily documentation of activities (progress notes), fire/ disaster drills, behavior support plans, monitor and track targeted behaviors, medications, ISP tracking, as well as other documentation and enter into documentation system per protocols.
  • Review medication administration reports for assigned homes and supervise individual with self-administered medications
  • Assist in creating a physical environment that lends to progress including house cleaning and laundry
  • Assist with the execution of ADLs
  • Communicate professionally and act respectfully with people we serve and support.
  • Participate in annual trainings to remain updated on skills and as needed based upon the behavior support plan.
  • Notify supervisor of individuals elopements or physically aggressive behaviors immediately.
  • Develop weekly/monthly menu with individual assistance.
  • Monitor the vehicles assigned to the homes for cleanliness and good working order.
  • Manage aggressive behaviors.
  • Other duties may be assigned.
