About The Position

Advocate for disability inclusion while identifying and creating new opportunities for classes and community involvement. Build rapport with individuals, their support teams, and community members. Create and update all required 245D support documents and attend and facilitate meetings with support teams. Train staff on best practices for delivering person-centered services.

Requirements

  • Associate degree & two years of experience in the human service field.
  • Fluent in English, verbal and written.
  • Proficiency in Microsoft Office Suite.
  • Must pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.).
  • Understand and communicate written and verbal directions.
  • Have regular attendance, reliability and punctuality is necessary to meet the needs of the business.
  • Are well organized with excellent attention to detail.
  • Work independently with excellent time management skills.
  • Work effectively with others to reach common goals and objectives.

Nice To Haves

  • Bachelor’s degree or a combination of four years of education and experience preferred.

Responsibilities

  • Providing services that assist people with disabilities in accessing the community through person-centered supports that lead to informed choice.
  • Documenting annual plans and creating Support Plan Addendums (SPAs), including but not limited to outcomes, goals, training strategies, evaluation plans, community resources, adaptive devices, instructional technology, therapies, and positive support transition plans.
  • Creating, implementing, and training staff to carry out outcomes and goals, training strategies, and positive support transition plans, while ensuring all required training documentation is completed as a designated coordinator.
  • Arranging and/or providing necessary transportation to and from community sites in collaboration with staff and team members, which may include driving individuals in a personal vehicle or a Lifeworks vehicle.
  • Arranging or providing additional support and assistance needed for individuals to complete personal care routines (eating, bathroom, and medical assistance), maintain and expand participation in activities, and strengthen their relationships and community involvement.
  • Building and sustaining community relationships to increase access to inclusive social opportunities for individuals.
  • Assisting individuals by collaborating with the Lifeworks employment team and other relevant services to support employment opportunities.
  • Managing billable time and ensuring all billing is completed accurately and in a timely manner.
  • Attending all required training and team meetings.
  • Following safety procedures and helping identify safety needs for oneself and others, and performing other duties as assigned.
