About The Position

Mozaic is seeking part-time Community Habilitation Direct Support Professionals. Community Habilitation (Com Hab) staff support individuals in becoming more actively engaged in their communities by offering opportunities to build connections, develop independence, and enhance daily living skills. Community Habilitation offers a variety of skill-building activities, such as cooking classes to promote meal preparation skills, as well as assistance with attending medical appointments, managing finances, and improving cleaning and organizational abilities. In addition to these supports, Community Habilitation also plans recreational outings to encourage social engagement and community participation. These outings may include trips to the zoo, museums, amusement parks, theater performances, movies, and other enjoyable activities.

Requirements

High School diploma or equivalent required.
Minimally, one year of experience working with people with developmental disabilities or an associate degree in human services or a related field is required.
A Valid Driver's License is required.

Responsibilities

Support individuals in becoming more actively engaged in their communities.
Offer opportunities to build connections, develop independence, and enhance daily living skills.
Provide assistance with skill-building activities such as cooking classes for meal preparation.
Assist individuals in attending medical appointments.
Help individuals manage finances.
Improve cleaning and organizational abilities.
Plan and accompany individuals on recreational outings to encourage social engagement and community participation (e.g., trips to the zoo, museums, amusement parks, theater performances, movies).
