Mary Cariola Center is seeking a Direct Support Professional II - Clinical Liaison. This role involves assisting with the orientation and training of new staff, supporting residential clinical staff, maintaining program supplies, and ensuring equipment is in good condition. The position requires attending medical/health appointments for residents, organizing and completing documentation, and reviewing it for accuracy. A key responsibility is facilitating resident participation in their individual program plans within home and community settings, implementing behavior support plans, and strategies for crisis intervention and prevention. The role also includes assisting residents with physical needs such as lifting, moving, positioning, and facilitating motor skills, as well as promoting self-care and personal hygiene. This includes assisting with bathing, toileting, dressing, and eating, and modeling appropriate behaviors. Housekeeping tasks like meal preparation, cleaning, laundry, and shopping are also part of the duties. Fostering communication through various methods, including sign language and electronic devices, is essential. Accurate and continuous documentation using PrecisionCare software and written forms is required, along with adherence to HIPAA confidentiality laws and the Code of Ethics for DSPs. The position involves attending staff meetings and case reviews, implementing fire, safety, and emergency procedures, and assisting with fire drills. Maintaining certifications in Medication Administration, SCIP-r, CPR, and First Aid is necessary, as is administering medications according to policy. All other duties as assigned are also part of the role.
