  • Ability to complete required training, including CPR, First Aid, and Medication Certification.
  • High School Diploma or GED required.
  • This position requires access to Electronic Personal Health Information (EPHI). Current HSA Policies and Procedures apply. It is the responsibility of the employee to read, understand, and comply with HSA policies and procedures that apply to protected information and security.
  • Questions about EPHI and HSA Policies should be directed to the HSA Security Officer.
  • Services provided by HSA will be welcoming, accessible, integrated, and responsive to the complex needs of people with co-occurring disorders/conditions.

Nice To Haves

  • Two years' experience working with people with Disabilities is preferred.
  • Supervisory experience is preferred.

Responsibilities

  • Support people in achieving their goals, preferences, and desired outcomes.
  • Advocate for individuals and ensure their rights and needs are respected.
  • Participate in developing and monitoring Individual Support Plans.
  • Provide leadership, supervision, and support to Direct Support Professionals.
  • Ensure staff follow individual plans, agency policies, and state regulations.
  • Monitor staffing, schedules, training, documentation, and service quality.
  • Communicate and collaborate with individuals, families, guardians, professionals, and team members.
  • Respond to and document incidents or changes in individual needs and coordinate additional support when needed.
  • Maintain accurate records and ensure required documentation and assessments are completed on time.
  • Help ensure a safe, clean, and supportive living environment.
  • Provide direct support and assistance as needed.
