About The Position

The Direct Home Care Worker provides supportive services to participants in various settings, including their homes, the community, during transport, and at appointments or the day health center. Responsibilities are detailed in the participant's Care Plan, which is based on assessments by a home care RNCM. The role focuses on assisting frail elders to maintain optimal physical and emotional comfort, promoting self-reliance and independence. The Direct Home Care Worker performs tasks outlined in the Care Plan, which may include personal care, assistance with activities of daily living, meal preparation, toileting, simple treatments, medication reminders, laundry, homemaking, and transportation support. Accurate and timely documentation of services provided is essential, and the RNCM and/or clinic nurse must be kept informed of participant progress. The Direct Home Care Worker must report customer service complaints, schedule deviations, and emergency situations promptly to their supervisor and operate within their scope of practice.

Requirements

Minimum High School Diploma or G.E.D. required.
Minimum of (2) years of relevant work experience.
Must have completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.
Participates in annual, required in-service training and screening, including infection control, TB testing, safety training, and BLS training.
Must possess the ability to establish and maintain interpersonal and interdepartmental relationships.
Must respect the physical, spiritual, and well being of participants and co-workers.
Must be able to organize and maintain flexible scheduling to coordinate job responsibilities.
Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.
Ability to relate to those participants with dementia and/or developmental disabilities.
Must be able to perform duties in an atmosphere of frequent interruption.
Must meet a standardized set of competencies (approval by CMS) before working independently.
Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Driving is required within PACE SEMI catchment area.
Must have reliable transportation available on a daily basis.
Must have current valid driver’s license.

Responsibilities

Provides supportive services to participants in home or community settings, during transport, and on appointments or at the day health center.
Assists in providing and/or maintaining optimal physical and emotional comforts to frail elders.
Assists participants and their families toward achieving maximum self-reliance and independence.
Performs care or services outlined in the individual participant’s Care Plan.
Performs a variety of tasks including personal care, assistance with activities, meal preparation/feeding, toileting/managing incontinence, simple treatments, shopping, medication reminders, laundry services, extended hours care, homemaking services/chores, and transportation/off-site appointment support.
Responsible for accurate and timely documentation of care and/or services provided on every visit and/or encounter.
Keeps the RNCM and/or clinic nurse informed of the participant’s progress.
Immediately reports all customer service complaints, deviations from set schedule and appointments to supervisor/manager.
Responds to emergency situations in accordance with established policies and procedures.
Works only within established scope of practice.
