The Direct Home Care Worker provides supportive services to participants in various settings, including their homes, the community, during transport, and at appointments or the day health center. Responsibilities are detailed in the participant's Care Plan, which is based on assessments by a home care RNCM. The role focuses on assisting frail elders to maintain optimal physical and emotional comfort, promoting self-reliance and independence. The Direct Home Care Worker performs tasks outlined in the Care Plan, which may include personal care, assistance with activities of daily living, meal preparation, toileting, simple treatments, medication reminders, laundry, homemaking, and transportation support. Accurate and timely documentation of services provided is essential, and the RNCM and/or clinic nurse must be kept informed of participant progress. The Direct Home Care Worker must report customer service complaints, schedule deviations, and emergency situations promptly to their supervisor and operate within their scope of practice.
