About The Position

The Direct Home Care Worker provides supportive services to participants in various settings, including their homes, community, during transport, and at appointments or the day health center. Their responsibilities are guided by the participant's Direct Home Care Worker Care Plan, which is based on assessments by a home care RNCM. The role involves assisting participants and their families to achieve maximum self-reliance and independence, maintaining their physical and emotional comfort, and performing tasks outlined in the individual participant’s Care Plan and relevant Care Guidelines.

Requirements

  • Minimum High School Diploma or G.E.D. required.
  • Must have a minimum of (2) years of relevant work experience.
  • Must have completed a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.
  • Participates in annual, required in-service training and screening, including infection control, TB testing, safety training, and BLS training.
  • Must possess the ability to establish and maintain interpersonal and interdepartmental relationships.
  • Must respect the physical, spiritual, and well-being of participants and co-workers.
  • Must be able to organize and maintain flexible scheduling to coordinate job responsibilities.
  • Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.
  • Ability to relate to participants with dementia and/or developmental disabilities.
  • Must be able to perform duties in an atmosphere of frequent interruption.
  • Must meet a standardized set of competencies (approval by CMS) before working independently.
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Driving is required within PACE SEMI catchment area.
  • Must have reliable transportation available on a daily basis.
  • Must have a current valid driver’s license.

Responsibilities

  • Assists in providing and/or maintaining optimal physical and emotional comforts to frail elders.
  • Assists participants and their families toward achieving maximum self-reliance and independence.
  • Performs personal care.
  • Assists with activities of daily living.
  • Assists with meal preparation/feeding.
  • Assists with toileting/managing incontinence.
  • Performs simple treatments.
  • Assists with shopping.
  • Provides medication reminders.
  • Performs laundry services.
  • Provides extended hours care.
  • Performs homemaking services/chores.
  • Provides transportation and off-site appointment support.
  • Responsible for accurate and timely documentation of care and/or services provided on every visit and/or encounter.
  • Documents transport and appointment details.
  • Keeps the RNCM and/or clinic nurse informed of the participant’s progress.
  • Immediately reports all customer service complaints, deviations from set schedule and appointments to supervisor/manager.
  • Responds to emergency situations in accordance with established policies and procedures.
  • Works only within the established scope of practice.
