Requirements
- Minimum 8th grade education required; High school diploma or GED preferred.
- Preferred: 3 months to 1 year of caregiving experience.
- Ability to read, write, and communicate effectively in English (interpreter required if not).
- Ability to follow care plans, policies, and procedures.
- Compassionate, reliable, and respectful with a professional appearance.
- Must be at least 18 years of age.
- Bilingual skills a plus.
- AHCCCS Direct Care Worker Training or current CNA/LNA certification (AZBN).
- Current CPR and First Aid Certification.
- DPS Level 1 Fingerprint Clearance (or obtain within 90 days of hire).
- APS Registry Check (initial and annual).
- Notarized Criminal History Affidavit (initial and every year after).
- Must have proof of TB clearance.
Responsibilities
- Help individuals with bathing, dressing, grooming, and hygiene.
- Assist with meal preparation, light housekeeping, laundry, ambulation, and transfers.
- Provide supervision and companionship.
- Offer temporary relief for family caregivers.
- Coordinate and accompany members on errands and appointments.
- Accurately document all services and use EVV per policy.
- Report changes in condition or incidents to the Member Care Supervisor.
