About The Position

The role of a Direct Care Staff, specifically as a Community Assistant (CA) for Community Access Individual (CAI) Services, involves providing supervision and support for individuals participating in community activities. The CA works with individuals to achieve the stated goals of their individual service plan under the CAI program. This includes assisting with life skills, approved goals by DBHDD, and independent living tasks. The service requires transporting individuals into the community for social activities related to their CAI goals. Additionally, the CA may provide in-home caregiving on a case-by-case basis. Creative Community Services (CCS) maintains personnel files for compliance, requiring specific documentation and completion of various trainings.

Requirements

  • Copies of social security card
  • Copies of driver’s license
  • Proof of liability insurance
  • Submitted TB skin test prior to working with an individual
  • Criminal records check prior to working with an individual
  • Fingerprinting to CSS prior to working with an individual
  • CRP first aid prior to working with an individual
  • CCS orientation prior to working with an individual
  • Complete IDD Supports training within 30 to 60 days of working with any CCS individual
  • Complete Medication training within 30 to 60 days of working with any CCS individual
  • Complete Personal Care Assistance (PCA) training within 30 to 60 days of working with any CCS individual
  • Complete CPR First Aid training within 30 to 60 days of working with any CCS individual
  • Complete Safety Care training within 30 to 60 days of working with any CCS individual

Responsibilities

  • Provide supervision, transportation and supports related to the CAI goal for the individual, including but not limited to life skills, approved goals by DBHDD, and independent living tasks.
  • Transport the individual out into the community to do social activities related to the CAI goal.
  • Document a daily log and goal tracking on the day the service is completed.
  • Submit daily documentation to the CA supervisor, the Family Consultant.
  • Submit a timesheet for services provided on the timeline given by their supervisor.
  • Work with the individual according to approved weekly hours from the supervisor.
  • Assist with caregiving in the home on a case by case basis.
