About The Position

This position provides community-based skill development services to members living in their own homes or independently managed residences. The role supports members in achieving goals outlined in their service plans by building skills that promote independence, self-sufficiency, and successful community integration. This position works collaboratively with members and their support teams to foster growth, stability, and overall well-being through trauma-responsive services.

Requirements

High school diploma or equivalent required
Valid Iowa driver’s license with an acceptable driving record and current automobile insurance required

Nice To Haves

Previous experience in human services preferred

Responsibilities

Work on service plan goals with the member as outlined in the service plan.
Provide skill development for members in their homes and communities, as outlined in the service plan. Utilize intervention steps and document member response and engagement, as well as staff intervention, in EHR.
Identify and utilize member’s safety plan and self-care plan when in crisis. Report to Case Manager.
Provide transportation to member, adhering to goal completion.
Acquire appropriate signed documentation from members as assigned by the Case Manager or Program Director.
Complete a SAFE-T with members when presenting or reporting suicidal ideation and communicate with Case Manager and/or Program Director.
Work with Case Manager and team to brainstorm skill-building activities.
Track member goal completion and report to Case Manager.
Track member behaviors and trends. Report to Case Manager.
Complete service documentation within 72 hours of service being provided.
Attend weekly team meeting and scheduled supervision.
Attend job-specific trainings as assigned and necessary to provide services.
Complete member surveys with members.
Maintain a valid driver’s license.
Complete Sanctuary Training within 90 days of hire and utilize tools during services (i.e., community meeting, SELF, safety plans, etc.)
Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.)
All other duties as assigned.
