About The Position

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking the following healthcare professional to join our team, supporting our Monticello, Spring Valley, Haverstraw and Peekskill health centers. We currently have the need for a Nutritionist at our Amenia and Peekskill locations. The Dietitian will perform individual counseling of patients referred by the medical personnel in all sites of Hudson River Healthcare. Performs job duties in compliance with systems and job specific standards.
Requirements
- Bachelor’s Degree
- 2-4 years Related Work Experience
- RD/ RDN and CDN with a valid CPR card
- Ability to communicate in English, both orally and in writing
- Ability to operate a motor vehicle and travel within the HRHC service site area
- Knowledge of nutrition principles as well as knowledge of nutrition during pregnancy and lactation, as well as infant and child feeding practices
- Knowledge of the geographic community serviced and resources
- Competence in nutrition assessment counseling
- Ability to collaborate with other member of Hudson River HealthCare staff in order to maintain high quality standards and achieve client satisfaction

Responsibilities
- Provides nutrition counseling to patients referred by clinicians
- Performs initial nutrition history on each patient
- Develops and implements a care plan based on the assessment of nutritional and health needs
- Provides instructions on a normal or special diet
- Schedules follow-up appointments and evaluates the reaching of goals
- Provides nutrition counseling for diabetic patients as part of diabetic care plan
- Provides nutritional education materials to patients to support counseling efforts
- Makes referrals to other health care providers and community services as appropriate
- Counsels individuals and families on nutrition and diet based on the particular needs of the individual or family as determined by their nutritional and health risk
- Attends staff meetings and in-service training sessions at both local and state levels
- Provides consultation into the development of program policies and procedures
- Provides direction and executes health education programs as needed
- Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served
- Must demonstrate knowledge of the principles of grown and development over the life span
