About The Position

Elderwood at Waverly is seeking a Registered Dietitian (license required) to join their clinical team in a full-time role supporting residents in long-term care and short-term rehab. This is a local opportunity to make a real difference in resident health and quality of life. The Registered Dietitian team member assists with nursing the health and well-being of our residents by being responsible for the nutrition assessments of our residents and monitoring their progress, keeping organized and cohesive documentation and conducting routine audits related to nutrition and food services.

Requirements

  • Bachelor’s degree in Nutrition, Dietetics, or related field
  • Registered Dietitian credential required
  • Active registration with the Commission on Dietetic Registration (CDR)
  • Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable
  • Strong communication and interdisciplinary collaboration skills
  • Ability to read, write, speak, and understand the English language at an intermediate or more advanced level.

Responsibilities

  • Complete nutrition assessments and reassessments for residents
  • Develop, implement, and monitor individualized nutrition care plans
  • Provide nutrition education and counseling to residents and families
  • Monitor residents’ nutritional status, weight trends, intake, and overall wellness
  • Maintain accurate nutrition documentation within the electronic medical record system
  • Participate in interdisciplinary care plan meetings
  • Partner with Dining Services to support appropriate meal planning and service
  • Serve as a nutrition resource for facility leadership and clinical teams
  • Support and mentor Diet Technicians and dietetic interns, as applicable
  • Conduct nutrition-related audits and performance improvement initiatives
  • Maintain compliance with CMS, DOH, and nutritional service regulations
  • Assist with staff education and in-service training programs
  • Support survey readiness and best practice initiatives
