Elderwood at Liverpool is seeking a compassionate and clinically strong Registered Dietitian to join their interdisciplinary team within their skilled nursing and rehabilitation community. Requirements
- Certified Dietitian-Nutritionist (CDN) License Required
- Registered Dietitian credential required
- Active registration with the Commission on Dietetic Registration (CDR)
- Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable
- Bachelor’s degree in Nutrition, Dietetics, or related field
- Strong communication and interdisciplinary collaboration skills
- Ability to read, write, speak, and understand the English language at an intermediate or more advanced level

Responsibilities
- Conduct nutrition assessments for long-term care and rehabilitation residents
- Develop and monitor individualized nutrition care plans
- Provide nutrition counseling and education to residents and families
- Collaborate with Nursing, Dining Services, Therapy, and medical providers
- Help ensure regulatory compliance and nutritional best practices
- Support overall resident wellness and quality outcomes
- Complete nutrition assessments and reassessments for residents
- Develop, implement, and monitor individualized nutrition care plans
- Provide nutrition education and counseling to residents and families
- Monitor residents’ nutritional status, weight trends, intake, and overall wellness
- Maintain accurate nutrition documentation within the electronic medical record system
- Participate in interdisciplinary care plan meetings
- Partner with Dining Services to support appropriate meal planning and service
- Serve as a nutrition resource for facility leadership and clinical teams
- Support and mentor Diet Technicians and dietetic interns, as applicable
- Conduct nutrition-related audits and performance improvement initiatives
- Maintain compliance with CMS, DOH, and nutritional service regulations
- Assist with staff education and in-service training programs
- Support survey readiness and best practice initiatives
