The Dietitian/Diabetic Educator provides evidence-based medical nutrition therapy, diabetes self-management education, and individualized nutrition counseling to inpatient and outpatient populations. Responsibilities include conducting nutrition assessments, developing care plans, educating patients, families, and healthcare staff, and collaborating with providers and the interdisciplinary care team to improve patient outcomes. This position partners with the Director of Clinical Support Services to support the overall operation of Nutritional Service, including assisting with the oversight of kitchen operations, kitchen inventory, regulatory compliance, food service quality, and nutrition standards. The Dietitian/Diabetic Educator maintains an outpatient clinic schedule to provide nutrition services for diabetes, weight management, malnutrition, chronic disease prevention, and other nutrition-related conditions. The Dietitian/Diabetes Educator also works with the primary care network and community partners to expand outpatient nutrition services and develop programs that meet community needs.
