About The Position

The Jicarilla Service Unit, also known as the Jicarilla Apache Health Care Facility, is located in Dulce, New Mexico. The Jicarilla Service Unit serves primarily the members of the Jicarilla Apache Nation with a service population of over 4,000 patients but also serves as other tribes as well. Nzhonachi'idle'ee translated in the Jicarilla Apache language as "a place to get well” is the tribal name of the clinic. The clinic is accredited by Accreditation Association for Ambulatory Health Care, Inc. (AAHC). The Clinic is a Patient-Centered Medical Home and a Certified Dental Home, JSU has dedicated staff working together with local community programs at all levels to focus on health promotion and disease prevention activities.

Requirements

  • Must have a current nutrition license in the state of New Mexico.
  • Must be registered by the American Dietetic Association as a Registered Dietitian Nutritionist.
  • Must have degree or diploma from a professional registered nutrition program approved by the legally designated State accrediting agency at the time the program was completed.
  • Must be a Certified Diabetes Educator.
  • Must have and maintain Current Basic Life Support (BLS).

Responsibilities

  • Leadership in weight reduction/exercise program as needed.
  • Conduct appropriate nutritional assessment and pertinent nutritional history.
  • Develop care plan for patients identified as nutritional risk.
  • Record clinical data in electronic patient's medical record.
  • Provide nutritional consultation on phone or home visit as appropriate.
  • Provide physician referred patients with therapeutic diet counseling.
  • Participate on multidisciplinary teams for care of patients with chronic illness as required.
  • Provide Nutrition consultation service to community related programs that are requesting nutritional consultation as clinic schedule allows.
  • Provide in-service training to Client staff as needed.
  • Comply with all Client Privacy Act, HIPAA, and Ethical regulations.
  • Submit monthly narrative reports to the Director of Public Health Nursing.
