As a Dietician, you will be responsible for facilitating the dietary needs of patients and providing education on the nutritional aspects of various chronic conditions. You will analyze the patients physical conditions and other health concerns and create a nutrition plan that meets their individual needs and dietary preferences. The Dietician will work collaboratively with our nurse practitioners, physicians, clinical support staff, other healthcare providers and external agencies. The dietician will be responsible for communicating with patients, guardians, and family members who may have chronic health conditions or health and wellness related questions to close gaps in care and to improve overall health and well-being. This Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $206,927,896.80 with 100 percent funded by CMS/HHS.
