About The Position

Astera Health currently has a Diabetic Educator opening at the Astera Health in Wadena, MN, as well as occasional travel to satellite locations. This position is Monday through Friday 8:00am-4:30pm, scheduled 64-80 hours bi-weekly.

Requirements

  • Current licensure as a Registered Nurse (RN) in the state of Minnesota.
  • Certified Diabetes Care and Education Specialist (CDCES) certification or ability to begin certification within 18 months of employment.
  • BLS

  • Ability to effectively operate standard medical equipment, electronic medical record software, and standard office equipment and software.

Responsibilities

  • Provides diabetes self-management education classes in both individual and group settings. Provides education related to diabetes and the disease process, medication, diet, foot care, prevention and care of complications, blood glucose monitoring and techniques of self-monitoring and injections.
  • Assists and plans community benefit events/support groups as assigned.
  • Maintains diabetes resource program accreditation through American Diabetes Association (ADA) as necessary, including gathering required statistical data; plans and executes annual diabetes advisory committee meeting.
  • Serves as a resource for hospital and medical staff regarding the care and treatment of patients with diabetes; collaborates with team members and medical staff to meet the needs of the patients and community.
  • Assists with policy development.
  • Provides clinical resources and consultation to staff and physicians; continues ongoing review of diabetes resource program curriculum to ensure patient education materials are up to date.
  • Assesses, monitors, and implements treatment plans for patients at various stages of the disease and provides ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required.
  • Documents assessments, interventions, and plans including recommendations, goals, progress notes, and discharge summaries.
  • Completes Transitional Care Management strategies for patients under the Diabetic Program.
  • Assists providers with diabetic medication refills.
  • Performs other duties as required or assigned.
