About The Position

The Indian Health Board of Minneapolis is a Federally Qualified Health Care Center and community clinic dedicated to providing access to quality health care and wellness services. They prioritize patient-centered, culturally sensitive care, respecting American Indian and Alaska Native traditions while embracing all cultures. The Diabetes Health Program Specialist will support the Special Diabetes Program for Indians (SDPI) and other health programs by managing patient and staff communication, planning events, and analyzing clinical data.

Requirements

  • Knowledge and respect of American Indian culture and other culturally diverse populations served by IHB.
  • Ability to implement projects from start to finish, thoroughly completing all phases and meeting deadlines.
  • Knowledge of or ability to be self-motivated to learn basic health screening guidelines.
  • Easily motivated, creative, engaging, and enthusiastic.
  • Associate's degree in a health-related field or equivalent experience in lieu of degree.
  • Experience working professionally with medical providers, clinic staff, community partners, and patients.
  • Reliable transportation.
  • Current license in good standing with a good driving record required.

Nice To Haves

  • 2+ years' experience preferred.
  • EHR experience is a plus.

Responsibilities

  • Participates as an integral member of the SDPI team, collaborating with various IHB staff to provide seamless patient care.
  • Collaborates with IHB's health programs to support patient care through various communication strategies, including scheduling meetings and patient appointments.
  • Takes meeting minutes and distributes them to applicable staff in a timely manner.
  • Completes inventory of purchased items on a regular basis for health program supplies and incentives, including clinical diabetes supplies and grant incentives.
  • Supports the diabetes clinic by purchasing and maintaining inventory for various health programs.
  • Maintains accurate and current quality measures for the team.
  • Collects and analyzes data from events, leading efforts in program audits, grant reporting, and grant activities.
  • Prepares for events and classes, and assists in meal preparation.
  • Works with IHB's SDPI team to order food and supplies for classes and events, following appropriate purchasing and reconciliation procedures, budget guidelines, and IHB's Healthy Foods guidelines.
  • Creates/updates flyers to promote health activities, events, and health education materials.
  • Collaborates with Care Coordinators on health program events.
  • Provides excellent customer service to patients, peers, leadership, and community partners.
  • Completes and submits all reports in a timely manner, ensuring accuracy and thoroughness (e.g., audits, grant reports, internal reports, etc.).
  • Assists with diabetes prior authorizations and contacts the pharmacy for 340B calls to ensure accuracy.
  • Sets up meetings and reserves rooms for all diabetic programs and other health program activities and/or events.
  • Performs other job-related duties assigned by supervisor.
  • Assists in workflow improvement related to job activities.
  • Attends staff and department meetings.
  • Takes meeting minutes and distributes them to staff.
