About The Position

This is a full-time preferred, 6-month contract position for an Advanced Practice Provider (APP) in Diabetes & Endocrinology at Alexander Park in Rochester, NY. The role involves outpatient care with low-moderate acuity patients, seeing 20-25 patients per day. The primary focus is diabetes care, including CGM analysis and insulin pump management. Charting is done via AI ambient scribe in EPIC. The position requires 2 years of minimum experience and a NY State License. Life Saving Certifications are also required. The work schedule is Monday-Friday, 8 am-5 pm, with potential for floating 1-2 days a week. Overtime and holiday pay is 1.25x the regular rate.

Requirements

• NY State License
• Life Saving Certifications (AHA or RQI)
• Minimum 2 years of experience
• Board Certified or Board Eligible
• Experience using EPIC
• No gaps of 30 days in work history
• Physician statement required if malpractice is involved (dismissed or not)
• Cannot accept Residents or Fellows

Nice To Haves

• Prior experience with CGM analysis
• Prior experience with insulin pump management

Responsibilities

• Provide outpatient diabetes care.
