About The Position

The Diabetes Educator will participate in special projects, community events, and healthcare professional education programs to increase diabetes awareness. This role involves completing documentation and record-keeping accurately and within expected time frames. The educator will provide ongoing therapeutic lifestyle coaching using approved resources and manage assigned caseloads. They will participate in practice changes resulting from performance improvement activities and support departmental quality standards. Additionally, the educator will serve as an advocate for care with community agencies, including pharmacies, insurance companies, and employers. They will educate patients and families about current research and assist with identifying and recruiting candidates for research studies. The role also involves seeking professional growth opportunities and participating in community activities such as educational lectures and support groups. The Diabetes Educator will provide diabetes instruction in group and individual sessions according to American Diabetes Association standards and departmental policy. They will obtain individual patient assessments, identify learning barriers, and provide appropriate resource referrals. Establishing individualized learning goals and educational plans in collaboration with patients and families is key. The educator will also prescribe appropriate nutrition plans based on patient needs and diagnoses, and evaluate patients upon completion of educational plans, providing follow-up as needed. Other duties as assigned.

Requirements

  • Advanced knowledge of diabetes concepts and skills including insulin pump therapy concepts as related to food intake.
  • Ability to provide teaching in all aspects of diabetes management and nutrition.
  • Ability to assess and interpret data relative to patient’s age and diagnosis.
  • Ability to provide comprehensive insulin pump training and glucose sensor training.
  • Ability to communicate effectively and organize daily activities.
  • Knowledge of computer programs such as Microsoft Outlook, Word, PowerPoint.
  • Ability to work with patients of various ages and backgrounds.
  • Ability to communicate fluently in English.
  • 1+ experience in diabetes specialty
  • 1+ experience in the field
  • Bachelor's degree
  • Basic Life Support - CPR Cert (BLS)
  • Certified Diabetes Care and Education Specialist (CDCES)
  • Registered Dietitian Nutritionist (RDN) OR Registered Nurse (RN)
