About The Position

The Diabetes Educator -Dietitian provides culturally sensitive education and consultation to patients, families and staff, as well as the professional and lay community. This includes information on nutrition, treatments, equipment operating, coping with a chronic illness and other related topics. The Diabetes Educator-Dietitian participates in team activities, staff education, professional organizations and other projects as assigned.

Requirements

  • Bachelor's degree in food/nutrition or dietetics and completion of a commission on Dietetics Registration (CDR) approved practice program
  • Minimum of two years in a health care setting working with patients with diabetes, preferably community based
  • Registered Dietitian (RD)
  • Licensed in Minnesota (LD)
  • Excellent verbal and written communication skills such as to communicate with a broad range of individuals

Nice To Haves

  • Certified Diabetes Care & Education Specialist (CDCES)
  • Knowledge of Spanish language and culture

Responsibilities

  • Provide culturally sensitive diabetes self-management training (DSMT), counseling and education to patients, both individually and in groups
  • Coordinate assessment, planning, implementation and evaluation of the education needs of the patient with diabetes
  • Initiate and adjust diabetes medications per the Hennepin Healthcare Certified Diabetes Care & Education Specialists Protocol
  • Order diabetes supplies and diabetes lab tests per Hennepin Healthcare Certified Diabetes Care & Education Specialists Protocol
  • Develop and implement nutrition care plans for patients based on individualized needs
  • Communicate with other members of the interdisciplinary health care team about the diabetes management and educational needs of patients
  • Maintain documentation in the electronic medical record (Epic) pertaining to diabetes education and management
  • Maintain Association of Diabetes Care & Education Specialists (ADCES) accreditation program records
  • Develop educational materials pertaining to diabetes using current evidence-based protocols
  • Work with clinic management and staff to improve diabetes care and measures within the clinic
  • Provide diabetes education to health care professionals and community groups
  • Serve as a preceptor to health professional students
  • Serve on committees or special projects related to the care of patients with diabetes
  • Maintain knowledge of the latest techniques, equipment and products available for diabetes treatment, care and education
  • Attain all agreed goals and objectives within specified time frames, as part of the organization’s overall mission
