About The Position

Certified Diabetes Care and Education Specialist – RN serves in an advanced role providing in-depth analysis of the patient with diabetes and providing specialized care and in-depth education for persons with diabetes and their families. Serves as a team member in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow up care for persons with diabetes. This is an outpatient diabetes educator position in an ADA-accredited clinic setting. Responsibilities include Diabetes Self-Management Training (DSMT), insulin pump education, and Continuous Glucose Monitor (CGM) education. Will consider candidates with relevant RN or RDN experience and credentials. Certified Diabetes Educator (CDCES) or Diabetes Educator experience preferred. Non-CDCES candidates will be considered if they have at least 2 years of healthcare experience as an RN or RDN and are willing to obtain certification. Non-CDCES candidates must sit for the CDCES certification exam within 2 years of hire.

Requirements

  • Diabetes Educator RDN Bachelor's Degree required
  • CDCES certification required within 2 years of hire
  • Registered Diabetes Educator RDN Bachelor's Degree required
  • Minimum 2 years of related experience required
  • Current CDCES/CDE certification required
  • Diabetes Educator RN Licensed Registered Nurse required
  • Minimum 2 years of relevant experience
  • CDCES certification required within 2 years of hire
  • Certified Diabetes Educator RN Licensed Registered Nurse required
  • Minimum 4 years of relevant experience
  • Relevant experience may include a combination of related work experience and education (Bachelor's Degree = 2 years of experience).

Responsibilities

  • Provides diabetes self-management education and training which includes evaluation of the individual’s skills, support, and barriers to care.
  • Assess general diabetes knowledge, evaluate physical findings, anthropometric measurements, and physical findings.
  • Teach person appropriate essential self-care behaviors to improve effective self-management of their disease. These may include healthy eating, being physically active, monitoring of blood sugar, medication administration and adherence, problem solving, healthy coping skills, and risk reduction behaviors.
  • Develops diabetes management plans appropriate to patient's immediate diabetes needs.
  • Reviews and assesses lab results and adapts clinical recommendations and care plan accordingly.
  • Serves as a resource for patient and family.
  • Documents in accordance with established standards.
  • Investigates and problem solves patient care issues with care team and facilitates change to promote quality outcomes.
  • Enhance quality of diabetes care through collaboration with other services and initiate referrals to appropriate support services.
  • Works within the multidisciplinary team for diabetes education and management and collaborates with other disciplines to identify and resolve patient care issues.
  • Conducts diabetes education through group classes as indicated.
  • Participates in departmental initiatives to meet strategic goals.
  • Precept students as indicated.
  • Mentor and train new staff members, colleagues, and other members of the care team as applicable.
  • Practices with a high degree of autonomy.
  • Evaluates evidence-based best practice and shares back to team including proposing adjustments to supporting reference materials, policies, etc.
  • Provides guidance to providers and other healthcare professionals.
  • Participates in multi-disciplinary care team and functions as the diabetes expert.
  • Collaborates with inter/intra professional team members to achieve department goals and optimize service.
  • Serve as Designated Point Person for new hires as indicated.
  • May include specialty program management and program evaluation outcomes.
  • Accountable for satisfying all job specific obligations and complying with all organization policies and procedures.
