Diabetes Educator - Registered Nurse (RN) or Registered Dietician (RD)
About The Position
This educator role specializes in Diabetes Education. Within this role candidates will educate complex diabetic patients across the continuum of care, coordinating and reviewing care plans with the bedside RNs. They will also be educating clinical teams on best practice standards. Hartford Hospital is one of the largest and most respected teaching hospitals in New England. Requirements
- MSN
- 3 years active clinical experience
- 1 year of acute care experience
- CDES Certification required within 18 months of hire
- BLS required
- Current CT RN license or CT RD license
Responsibilities
- Facilitates training, education and development by assessing, planning, developing, implementation, coordination and evaluation of projects and programs for staff, faculty, patient populations and the community.
- Educates complex diabetic patients across the continuum of care.
- Coordinates and reviews care plans with the bedside RNs.
- Educates clinical teams on best practice standards.
