About The Position

The Diabetes Educator - Endocrinology position at Erlanger Medical Center is responsible for the direct and/or indirect care of patients under the supervision of a Physician or Nurse Practitioner/Physician Assistant. This role is integral to a team-based approach to care, where team members are trained to perform at their highest level according to State of Tennessee/Georgia guidelines. The position involves coordinating patient care, supporting patients, families, and caregivers in self-management, self-efficacy, and behavior change, and managing the practice's patient population. The educator will also participate in the practice's quality improvement process and performance evaluation. Key responsibilities include teaching diabetes self-management in both individual and group settings, establishing and coordinating diabetes self-management training for patients (primarily adults), participating in client-centered case management, and coordinating multidisciplinary plans of care. The employee must demonstrate the knowledge and skills necessary to provide age-appropriate care, assess patient status, interpret information relevant to age-specific needs, and provide care according to departmental policies. Additionally, the employee must possess the skills to educate adult learners, teach diabetes self-management to individuals and classes, help manage outcomes, and assist with other assigned diabetes education activities.

Requirements

  • Certified Diabetes Educator or certification eligible RN, RD, Pharm D or Social Worker.
  • Graduate from an approved program in dietetics/nutrition or a Registered Nurse or Certified Diabetes Educator.
  • CDE eligible Pharm D or Social Worker.
  • A minimum of three years of progressive experience in a hospital or clinic setting required.
  • Experience in Insulin Pump Education.
  • Current professional registration required.
  • BLS prior to going to the unit/ department and maintained going forward.

  • Experience in teaching diabetes education.

Responsibilities

  • Demonstrate the knowledge and skills needed to provide education to adult learners.
  • Teaches diabetes self-management to individuals and classes.
  • Helps manage outcomes.
  • Assist with other activities that promote and encourage diabetes education as assigned.
