About The Position

The Diabetes Education Coordinator is the lead educator for the Diabetes Self-Management Education and Support (DSMES) program. This position is responsible for overseeing program content, ensuring compliance with Association of Diabetes Care and Education Specialists (ADCES) standards for recognition, monitoring program and patient data, and communicating / coordinating with the diabetes education team. The Diabetes Education Coordinator is designated on record with the ADCES and the primary contact for ADCES. This is a part time, hourly position working 32 hours per week, Monday through Friday.

Requirements

  • Completion of accredited Registered Nurse program or Bachelor’s Degree in Human Nutrition and Foods or similar.
  • Registered Nurse or Registered Dietitian in good standing, licensed in the State of Washington and/or the national CDR for Dietitians
  • Certified Diabetes Care and Education Specialist certification to be obtained within 1 year of hire
  • Previous experience working within a DSMES program preferred.
  • Two to five years clinical experience, including experience in patient care and program management.
  • Experience in teaching to a diverse population.
  • Intermediate computer skills.
  • EPIC experience preferred.
  • Basic Life Support Healthcare Provider (HCP) level to be completed within six (6) months of hire.

  • Previous experience working within a DSMES program preferred.
  • EPIC experience preferred.

Responsibilities

  • Reviews and maintains DSMES curriculum in compliance with ADCES standards for recognition.
  • Provides input and standards in formulating policy and procedures for the organization.
  • Coordinates and communicates with Samaritan Clinic departments to deliver education and update content as needed.
  • Works with Nutrition Services Director to establish program objectives and budgetary needs, and troubleshoots problems as needed.
  • Conducts patient assessments, 1:1 education, and follow-up for diabetes patients, following the ADCES standards for recognition.
  • Works with providers to coordinate medical and personal management of the patient.
  • Maintains required patient data for program monitoring and for ADCES audit-readiness.
  • Collects patient-participant feedback and makes or recommends program adjustments as necessary.
  • Conducts Continuous Quality Improvement Plans, collects data, and reports back to ADCES at annual review.
  • Performs other reasonable, position-related tasks as requested by management.
  • Participates in department meetings and/or across the organization as applicable.
  • May provide education to inpatients of Samaritan Healthcare as needed/requested.
  • Maintains 15 CEU hours annually of professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest information in Diabetes Education.
