About The Position

The DIABETES EDUCATION CDE Nurse is responsible for providing direct diabetes self-management education. This role involves conducting patient assessments, directing referrals for screenings, and contributing to program assessment and goal setting. The nurse will also collect and compile statistical data for accreditation, participate in quality assurance and staff development, and act as a liaison to physicians and other healthcare professionals. Additionally, the position facilitates referrals, markets the program to consumers, and assists in developing patient education activities focused on health promotion and wellness based on community needs.

Requirements

  • High school diploma or equivalent required.
  • Graduate of an accredited school of nursing.
  • Certification in Diabetes Education.
  • Must be able to communicate effectively in English, both verbally and in writing.
  • Registered with the BNE for the state of Texas.
  • CPR Certification.
  • Has a working knowledge with audio-visual equipment, personal computer, and Home Glucose meter download software.
  • Must be able to communicate verbally and written in a clear, understandable, and legible manner.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult situations.
  • Must be able to adapt and function in a changing environment.
  • Must be flexible and handle a large volume of information in an organized manner.

Nice To Haves

  • Spanish Speaking preferred.
  • Certification as a Diabetes Educator preferred.

Responsibilities

  • Providing direct diabetes self-management education.
  • Conducting patient assessments upon program entry and directing referrals for screenings.
  • Assisting in the assessment of the program and its goals.
  • Collecting and compiling statistical data required for accreditation.
  • Participating in quality assurance and staff development activities.
  • Functioning as a program liaison to physicians and other healthcare professionals.
  • Facilitating referrals and marketing the program to consumers.
  • Assisting with the development of other patient education activities that focus on health promotion and wellness based on community needs.
  • Performing patient assessment to department standards with special attention to teaching and educational abilities of patient and SO.
  • Performing targeted assessment/reassessment on subsequent patient visits.
  • Ensuring documentation of assessment and progress notes/flow sheets are thorough and timely.
  • Formulating a goal-directed plan of care that addresses patient’s educational needs in conjunction with the physician, integrating the plan of care with potential needs at home, and formulating discharge plan of care.
  • Implementing care in a knowledgeable, skillful, consistent, and continuous manner, establishing priorities of patient care based on essential patient needs and available resources.
  • Keeping physician, nursing, and other medical personnel aware of patient progress toward goals and needs for modification in plan of care.
  • Judging the effectiveness of care on each patient visit, evaluating patient response to care provided and patient/SO self-care responsibilities in relation to education, and formulating and documenting results of follow-up evaluation/visits.
  • Assessing the patient’s readiness to learn, level of understanding, and individual goals, planning an individualized educational approach to achieve goals, implementing teaching protocols for helping patient/SO learn, evaluating patient’s understanding and goal achievement, and documenting the educational process in the medical record.
  • Assisting in formulating patient teaching protocols for patients served.
  • Working in a collaborative manner with the physician, nursing, and other medical personnel to teach the patient/SO self-care as is possible.
  • Participating in Quality Improvement as requested by the Coordinator of Patient Education.
  • Initiating and monitoring insulin pump usage, and effectively educating patients/SO and managing usage, using established protocols.
  • Performing all other tasks/responsibilities as necessary.
