Requirements

• DDS degree from an accredited college or university
• Licensed to practice dentistry in Wisconsin
• DEA registration
• Mandatory vaccination for all employees, employed providers and volunteers. Proof of vaccination must be submitted prior to orientation.

Responsibilities

• Providing and ensuring the provision of preventative and restorative dental care that is consistent with accepted professional standards.
