About The Position

The Decentralized Clinical Pharmacist (DCP) at Children's Hospital of Philadelphia (CHOP) will provide pharmaceutical services to improve drug usage and therapeutic outcomes. This includes advising medical professionals on drug therapy, evaluating pharmacokinetic levels, providing medication counseling, and contributing to adherence to the CHOP Formulary. The DCP will review and process medication orders using the hospital electronic health record (EHR) to ensure medication appropriateness and supervise pharmacy technicians and interns. The role involves developing and maintaining clinical pharmacy practice for optimal pharmaceutical care, participating in clinical rounds, reviewing patient pharmacotherapy, communicating with prescribers and nurses, and ensuring compliance with institutional guidelines and regulatory standards. Additional responsibilities may include experiential education of pharmacy students and residents, and participation in research or education in pediatric pharmacy.

Requirements

  • Licensed Pharmacist (Pennsylvania) - Pennsylvania State Licensing Board - upon hire
  • PGY1 Pharmacy Residency
  • Extensive knowledge of drugs, medical therapeutics, pharmacokinetics, diagnosis and disease states
  • Ability to establish and maintain working relationships with various members of the health care teams
  • Ability to communicate clearly in oral and written form
  • Knowledge and comfort in using Electronic Health Records and other Pharmacy technologies

Responsibilities

  • Provide pharmaceutical services to improve drug usage and therapeutic outcomes.
  • Advise medical professionals on drug therapy, including inherent toxicity, side effects, and appropriate dosing.
  • Evaluate pharmacokinetic levels.
  • Provide medication counseling to patients and families.
  • Contribute to the adherence of the CHOP Formulary.
  • Review and process medication orders using the hospital electronic health record (EHR) to ensure appropriateness in dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state.
  • Supervise pharmacy technicians and interns to ensure accurate and timely preparation and distribution of pharmacy products.
  • Prepare and distribute pharmacy products when necessary.
  • Develop and maintain a consistent clinical pharmacy practice that provides optimal pharmaceutical care and support.
  • Participate in clinical rounds.
  • Review patient's pharmacotherapy to ensure medication appropriateness.
  • Communicate with prescribers and nurses to recommend alternatives for inappropriate therapy or substitutes for non-formulary items.
  • Ensure and promote compliance with the institution's formulary system and other guidelines or protocols.
  • Provide indicated pharmacokinetic monitoring.
  • Deliver consultative services regarding drug therapy to physicians, nurses, pharmacists, and other health care professionals.
  • Ensure and maintain compliance with regulatory standards for medication management (e.g., Joint Commission and CMS).
  • Perform pharmacist functions to ensure accurate and timely medication delivery.
  • Ensure compliance with federal, state, and department standards and regulations for compounding (USP 795/797/800), CMS Conditions of Participation, FDA Compounding Guidance, and Joint Commission standards.
  • Participate in hospital and departmental Continuous Quality Improvement (CQI) activities.
  • Utilize Lean Principles to analyze and resolve problems systematically.
  • Participate in departmental LEAN Projects (RPI / 5S).
  • Work overtime to allow co-workers to participate in department RPI.
  • Provide pre- and post-project feedback and support sustainment measures.
  • Achieve patient safety and satisfaction through the use of lean principles: 5S, Elimination of Waste, JIT, Stand Work, Quality.
  • Promote and comply with Standardized Procedures and Standard Work.
  • Identify waste in ownership process and recommend solutions for implementation.
  • Correct abnormal conditions.
  • Experiential education of pharmacy students (IPPE and/or APPE) and residents.
  • Participation in pediatric pharmacy or subspecialty focused research or education.
