About The Position

Functions as a member of Valley HealthCare System’s Mental Health Day Treatment and Residential Supports Programs, serving under the Program Manager and supervising Community Skills Trainers. The Day Treatment Supervisor assumes primary responsibility for the development and implementation of individual program plans for person served through regularly scheduled day treatment activities and individual supports. The Day Treatment Supervisor is responsible for program development as well as continuous evaluation of program effectiveness and appropriate changes. The Day Treatment Supervisor develops, monitors, evaluates, and documents plans of intervention and instruction as specified in the consumers’ treatment plan. The Day Treatment Supervisor provide ongoing regularly scheduled pseudo-therapeutic support, instruction, and activities to consumers with mental illness, intellectual/ development disabilities, and/or substance use disorder diagnoses for the purpose of increasing consumer wellness recovery and daily living skills.

Requirements

  • Ability to perform Essential Duties as outlined below.
  • BA/BS in Human Service Field
  • Valid Driver’s License
  • Ability to obtain and maintain CPR, First Aid, and Mandt Certifications.
  • Ability to comply with Consumer’s Rights.
  • Ability to work independently with minimal supervision
  • Ability to advocate on behalf of clients
  • Knowledge of agency, state, and federal guidelines.
  • Minimum 1-year experience working with individuals with Intellectual/ Developmental Disabilities, Mental Illness and/or Substance Use Disorders
  • Ability to comply with Valley’s and Division departmental safety procedures.
  • Ability to read, write and speak the English language.
  • Basic Computer Skills (Typing, Microsoft Word)
  • Ability to document services in an Electronic Medical Record (EMR)

Responsibilities

  • Participate in treatment plan meetings, staffing, and clinical meetings
  • Evaluate individuals and/or current plans of intervention or instruction
  • Train staff members on implementation of person-specific plans of intervention or instruction and data collection for Day Treatment Program
  • Evaluate/monitor effectiveness of plans of intervention or instruction through phone contact, necessary home visits, and day treatment activities.
  • Implement treatment/behavior/plans in compliance with Valley HealthCare System’s and State guidelines.
  • Engage consumers in achieving their daily goals and objectives through active treatment.
  • Maintains Day Program in compliance with budget standards.
  • Approves employees’ timecards, time off requests, and expenses
  • Supervise day program employee(s) through appropriate training, evaluation, orientation, scheduling, assignment and delegation of duties.
  • Assure that the safety, health and well-being of consumers are always maintained on or off site.
  • Adhere to Valley HealthCare System’s Values & Standards of Performance & Ethical Conduct, in addition to any professional code of ethics
  • Promote the dignity of the client through service provision.
  • As a role model, group leader, and motivator, the Day Treatment Supervisor will supervise Community Skills Trainers to facilitate and assist consumers in achieving goals of wellness and daily living skills (i.e. grooming, hygiene, nutrition, cooking, cleaning, taking medications appropriately, etc.).
  • The Day Treatment Supervisor will serve as a Community Skills Trainer in the absence of Community Skills Trainers
  • Documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies.
  • Employee ensures that documentation (such as Direct Services Progress Notes, Client Transportation Progress Notes, etc.) are completed accurately and timely and will reflect services the client(s) has received services as appropriate.
  • Complete Electronic Medical Record (EMR) Documentation during each daily episode of care, notating, clinically, client’s progression/regression in each of the clients’ individualized treatment goals identified in Master Treatment Plan
  • Completes and submits program specific documentation in a timely and accurate manner.
  • Monitor, maintain and use appropriately, all equipment and supplies.
  • Transportation of consumers to appointments, activities in the community, and to/from Program site, using a company vehicle.
  • Support and help in the training of new employees.
  • Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned.
  • OTHER DUTIES AS ASSIGNED.
