About The Position

Reporting to the Day Habilitation Asst. Director, the Day Hab program support coordinator responsibilities include supervision of assigned Day Habilitation Disabilities Specialist including performance evaluations. Ensure staff action plans, goals and all other plans are implemented with the individuals and that all staff are trained in these plans. Responsible for checking precision care to ensure all required meetings have been held, all timelines have been met and all required documents have been uploaded to Pcare. Ensure all OPWDD/Medicaid requirements/standards for meetings and billing are met.

Requirements

  • High School Diploma required
  • Minimum one-year experience working with individuals with developmental disabilities.

Nice To Haves

  • Bachelor's degree from four year college or equivalent​ preferred.
  • Valid NYS driver's license preferred but not required.

Responsibilities

  • Program Operations Participate and attend required meetings such as quarterly medical and clinical meetings and as needed, Day Program and Life Plan meetings. Ensure recommendations are occurring as prescribed
  • Oversee coordination and follow-up of program participants’ needs and goals. Ongoing observation of activities, both at the program and in the community.
  • Maintaining ongoing direct contact with all assigned program participants to ensure comprehensive knowledge of their behavioral, social, and functional needs.
  • Create and maintain a projected schedule for semi-annual and Annual Life Planning Team meetings in conjunction with care managers, day program team and/or employment representatives, Respite, residence, advocates and/or involved family members.
  • Ensure Day Habilitation Support Professional Specialist complete written minutes to accompany Staff Action Plan and reflect progress towards completing goals and supports and any changes made to the plan.
  • Ensure the DHSPS, working with the Day Habilitation support Professional (DHSP), and the individual are trained and are implementing goal-related and valued outcome-related activities, both at the program center and in the community.
  • Ensuring that support is provided with the appropriate level of supervision and consideration for each person’s individualized needs and routine.
  • Reviewing, correcting, and approving staff action plan including all goals/valued outcomes and community inclusion activities developed and written by the DHSPS.
  • Certify Staff Action Plan on a semi-annual and Annual basis or as necessary.
  • Ensure that Staff Action Plan and goals and supports identified correspond with at least two CQL Valued Outcome as identified on the individual’s Life Plan document.
  • Ensure that each individual has a current and accurate Life plan and LCED/LOC along with the proof of distribution of the plan on file and uploaded to Pcare.
  • Ensure Day Hab support professional Specialist maintains case record documentation in accordance with HeartShare Policies, Protocols and Practices on at least a quarterly basis.
  • As needed serve as a liaison with family members, guardians, and outside agencies.
  • Review and certify all monthly summaries ensuring that they are completed on time, ensuring they are reflective of progress.
  • Review with DHSPS as needed, each assigned program participants behavior plan, medical and Staff action plan to determine if the Program Planning Team needs to meet to discuss modifications to existing goals/valued outcomes ect or implementation of new ones.
  • Ensure that DHSPS are completing the appropriate and timely collection of services information, including behavioral data and community inclusion calendars
  • Following all agency, state, and federal regulations to maintain program compliance.
  • Completing all written reports and documentation on a timely basis
  • Ensures with the DHSPS; program participants are always clean, safe and provided with an appropriate program environment.
  • Ensure staff are trained in following the LOS for each person and ensure that there is adequate coverage for each group.
  • Attending all intake and admissions meetings.
  • Prior to admission, writing initial Staff Action Plans; thereafter, ensures the Day Habilitation support professional Specialist on both an annual and semi-annual basis, including input from the program participants, family members, day services team members, and Care Coordinators as required.
  • Reporting all incidents, including allegations of abuse and neglect and serious incidents, to either Day Hab Director or Asst. director immediately upon discovery and ensuring that the Justice Center Hotline is called as appropriate.
  • Assist in filing 147 incident reports as necessary.
  • Serving as an advocate on behalf of program participants.
  • Providing in-service training for staff as required, including training in writing and implementing goals as well as other areas deemed appropriate by either the Day Hab Director or assistant director.
  • Participating in in-service training and on-site workshops is required for professional growth and development.
  • Attending external training, workshops and/or Program Planning Team meetings at direction of direct supervisor or program management.
  • Maintaining certification in CPR/First Aid and SCIP, as required.

  • Undertaking any additional tasks and initiatives as assigned within the scope of the Day Hab site director or asst. director.
  • Documentation and Fiscal Secure documents for submission including but not limited to Scripts, Informed Consent Forms, PPT sign offs etc.
  • Along with the Day Habilitation Support Professional Specialist, review Pcare daily to ensure that staff are documenting services as needed, Create and submit monthly community inclusion calendar for all groups
  • Timely submission of monthly report to QA staff and uploaded to Pcare. (this document is created and maintained by the Day Habilitation support coordinator and reviewed by Director or assist. Director )
  • Staff Supervision and Development Participating in the interviewing, hiring and initial in-service orientation of the Day Habilitation Support Professional Specialist.
  • Providing ongoing and timely supervision of assigned staff, including individual monthly staff supervision, quarterly observations and annual Evaluations are completed.
  • Complete probationary evaluations within identified timeframe for new Day Service Support Professional Specialist
  • Approve staff evaluations by DHSPS.
