Requirements
- Must possess a high school diploma or GED
- 3 years work experience in a healthcare related field
- Experience with customer service
Responsibilities
- The medication delivery concierge will be responsible for coordination of medication delivery to patients discharged home.
- The concierge will work closely with staff in the following departments, other readmission reduction staff, case management, outpatient pharmacy staff, and the care team to facilitate a smooth delivery of medications.
- The concierge will maintain databases to ensure accurate documentation of the medication delivery program.
- Concierge staff will assist with other readmission reduction strategies and quality related projects as assigned.
