About The Position

Crosstown Clinic is seeking Registered Nurses/Registered Psychiatric Nurses to join their harm reduction-driven team. Crosstown is the first clinic in North America to offer medical-grade heroin (diacetylmorphine) and the legal analgesic hydromorphone within a supervised clinical setting to chronic substance use patients. The clinic is supported by an on-site pharmacy and an interdisciplinary care team composed of doctors, nurses, social workers, and substance-use disorder counsellors. Additionally, they offer life-skills counselling, housing referrals, and direction to legal assistance. Requirements

  • Current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
  • Ability to communicate effectively both verbally and in writing.
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
  • Ability to work as a member in an interdisciplinary team.

Responsibilities

  • Provide compassionate, evidence-based nursing care to clients with substance use disorders within a harm reduction framework.
  • Monitor client health and wellbeing before, during, and after treatment, respond to medical or behavioural crises, and collaborate with the interdisciplinary team to ensure safe and effective care delivery.
  • Assess, plan, implement, and evaluate individualized care plans using clinical judgment and best practices.
  • Maintain accurate and timely documentation of assessments, interventions, progress notes, and discharge plans in accordance with professional standards.
  • Educate and coach clients and families on health-promoting behaviours, harm reduction strategies, and treatment options.
  • Advocate for clients’ dignity, rights, and access to care, ensuring services are provided with compassion and respect.
  • Work closely with physicians, social workers, counsellors, and other team members to coordinate care and facilitate continuity across services.
  • Provide clinical guidance to support staff and contribute to quality improvement, research, and educational initiatives within the clinic.
  • Review patient/resident status through methods such as reviewing the patient/resident chart and other written information, patient/resident observation, interviewing patient/resident and family members, completion of questionnaires, collateral information and consultation with peers and other members of the health care team.
  • Assess patient/resident problems and needs, plan, implement, evaluate and prioritize nursing care and nursing interventions and develop the plan of care for patients/residents using clinical judgement and evidence-based practice.
  • Provide nursing care according to identified policies and procedures, technology and PHC standards.
  • Monitor patient/resident progress, evaluate outcomes, reassess if goals are not met, and modify plan of care based on evaluation.
  • Record observations and report problems and/or changes in patient/resident condition to designated staff.
  • Provide direct nursing interventions for management of crisis and life threatening situations.
  • Document information including assessments, plan of care, patient/resident status and progress and discharge plans in a manual and/or computerized environment.
  • Accompany patients in medical transport vehicles as required by the employer.
  • Identify and respond to patient/resident and family learning needs, provide patient educational and reference materials, incorporate individual and/or group teaching activities into the care plan and nursing interventions, and/or make referrals to other identified resources.
  • Lead group patient/family sessions as required and coach patient/resident/family to develop and maintain health promoting behaviours within the context of the care issue.
  • Provide direction to other staff such as Care Attendants and Licensed Practical Nurses with regard to patient/resident care and facilitate effective team work, including collegial sharing of information, demonstrating and/or providing guidance on related tasks, providing incidental or ad hoc input into staff performance, ensuring staff are aware of assignments and reassigning clinical/health related tasks.
  • Participate in nursing orientation and share clinical knowledge with other nursing staff, nursing students and other members of the interdisciplinary team.
  • Participate in and attend in-service presentations and other educational programs as required.
  • Participate in planning, implementation and evaluation of patient/resident care policies, procedures, education materials, and best practices through methods such as taking part in program and quality improvement activities including nursing practice council, clinical pathway development, research and education and other committees.
  • Support nursing research through methods such as identifying areas for possible clinical nursing research projects, adapting research to practice and participating in research activities to enhance nursing practice.
  • Facilitate discharge plans and liaise with families and community resources agencies.
  • Advocate for the patients/residents, to protect and promote their rights to dignity, respect, privacy, confidentiality, autonomy and access to information.
  • Perform other related duties as assigned.
