About The Position

The Nurse Practitioner provides primary care services to children and adults served by Woods System of Care and its affiliates, community members, and employees. This also includes individuals with intellectual and developmental disabilities, autism, and traumatic brain injury. In this role, the Nurse Practitioner provides preventive and acute care, chronic disease management, health education, care coordination, and other clinical services within the scope of practice. The Nurse Practitioner is responsible for timely and accurate documentation of client encounters in the electronic health record (EHR) in accordance with established standards and participates in Interdisciplinary Team (IDT) meetings as needed. The Nurse Practitioner will provide services in Pennsylvania and New Jersey.

  • Bachelor’s Degree in Nursing (BSN), Master’s Degree in Nursing (MSN) and/or Doctor of Nursing Practice (DNP) preferred.
  • 3 or more years of clinical experience as a Nurse Practitioner.
  • Experience working with vulnerable populations.
  • License as a Nurse Practitioner in Pennsylvania and New Jersey.
  • Board Certification as a Nurse Practitioner in family practice.
  • Pennsylvania and New Jersey Registered Nurse license.
  • DEA certification.
  • BLS certification.
  • Required Act 48 continuing education course work for Nursing license.
  • Required State and Federal clearances.

Responsibilities

  • Provides primary health care services, including medication management, and prescribes medications as appropriate to individuals served by the Medical Center.
  • Evaluates and orders appropriate lab/diagnostic tests, chronic health assessments, and coordinate all follow-up care as needed.
  • Provides mandated screenings as well as screenings for common chronic conditions.
  • Manages acute and chronic health problems using evidence-based protocols.
  • Documents all patient services in a timely manner into the electronic medical record.
  • Provides first aid for illness or injury to patients and documents assessment, diagnosis and treatment.
  • Establishes health care plans of individuals served by The Medical Center and updates them as needed.
  • Completes peer review reports as required.
  • Performs initial or biennial employee physicals and drug testing.
  • Ensures that all required reports are properly prepared and submitted.
  • Performs all documentation for billing in accordance with agency policy.
  • Works collaboratively with Medical Director, Health Center Leadership and their designees to comply with the Medical Center’s policy related to billing, credentialing, and reporting.
  • Facilitates close communication with community partners, RL Health leadership, management and other key staff for care coordination as appropriate.
  • Attends required meetings.
  • Provides coverage for physicians and other NPs/PAs employed by or contracted with the Medical Center as necessary.
  • Represents the organization and its partners externally.
  • Collectively participates in on call after hours coverage for medical services.
