Serving communities across Colorado, we offer equitable and creative “one-stop” behavioral health services, including in-home mental health and substance use treatment, crisis stabilization, early childhood support, intensive outpatient programs, assertive community treatment, medication management, and wrap-around support. Our services include intensive care management, Crisis Stabilization programming, Assertive Community Treatment (ACT), Child First, integrated co-occurring Intensive Outpatient treatment, individual and family therapy, medication management (including MAT), peer support, supported employment/housing, and holistic recovery services for mental health and substance use disorders.

Requirements

  • Master’s degree in Counseling, Social Work, or related field.
  • Colorado licensure or eligibility for licensure (LCSW, LPC, LMFT, licensed clinical psychologist, or equivalent).
  • Experience working with culturally diverse individuals and families, trauma, group facilitation, and crisis stabilization supports.
  • Knowledge and experience in mental health and substance use assessments and consultation.
  • Ability to provide interventions within diverse home and community settings.
  • Strong written and oral communication skills.
  • Familiarity with HIPAA, 42 CFR, protected health information, and confidentiality.
  • Commitment to the organization's mission, vision, and goals.
  • Has a confirmed master’s degree in counseling, social work, or related field while working towards clinical licensure.
  • Licensed or licensed-eligible (must be able to obtain a preliminary license within six months of employment)
  • All licenses must stay up-to-date and valid during employment.
  • A valid driver’s license and proof of automobile insurance are required.

Nice To Haves

  • Ability to speak a second language (Spanish, ASL or other), highly valued.
  • Master’s or Doctoral level mental health provider (e.g., LCSW, LMFT, LPC, licensed clinical psychologist, other) is preferred.

Responsibilities

  • Provide outpatient therapy to children, adolescents, adults, and families.
  • Deliver services in homes, schools, community settings, and office locations.
  • Complete biopsychosocial assessments, treatment plans, and clinical documentation.
  • Provide individual, family, and collateral therapy sessions.
  • Utilize evidence-based practices appropriate to client needs.
  • Coordinate care with schools, primary care providers, child welfare agencies, probation, and other community partners.
  • Participate in multidisciplinary treatment planning and case consultation.
  • Maintain compliance with agency, payer, and regulatory documentation requirements.
  • Participate in clinical supervision, staff meetings, and ongoing professional development activities.
  • Support additional community-based behavioral health programming as assigned.
  • Provide approximately 12 hours per week of in-person behavioral health services within the San Miguel County Jail.
  • Conduct mental health and substance use screenings and assessments.
  • Complete GAIN assessments and other JBBS-required documentation.
  • Conduct suicide risk assessments and crisis intervention services.
  • Provide brief individual counseling and therapeutic interventions.
  • Develop individualized treatment and transition plans.
  • Coordinate psychiatric evaluations, medication continuity, and MAT referrals as appropriate.
  • Collaborate with jail staff, medical providers, probation officers, courts, and community treatment providers.
  • Facilitate discharge planning and community reentry coordination.
  • Complete Civicore documentation and JBBS reporting requirements.
  • Participate in JBBS program meetings, quality improvement initiatives, and compliance activities.
  • Provide urgent behavioral health interventions to individuals under mental health holds and those requiring support as they transition back into the community.
  • Facilitate crisis stabilization programming using an evidenced based model.
  • Transition clients to ongoing services conducted by in-home clinical teams as appropriate.
  • Work as part of a multidisciplinary team and serve as a team leader.
