Requirements

• Crisis Social Worker Emergency Department (Per Diem)

Responsibilities

• Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter.
• Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines.
• Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program.
