About The Position

Provide crisis and other outpatient services to children, youth, families and adults for the purpose of improving the quality of life for the person/family served. This position is supervised by the Program Supervisor or other identified member of management. The role promotes a warm and engaging organizational culture in line with NorthCare values, trauma informed care, and health and wellness. Responsibilities include crisis management, coordination of intervention and referrals, and providing integrated services that address behavioral healthcare, physical healthcare, and social needs. The therapist will also provide brief evidence-based counseling, group therapy, and supportive services as needed. Additionally, the role involves performing screenings, intake interviews, treatment plan reviews, and working as part of a multi-disciplinary team. Maintaining clinical records and professional competency is also a key aspect of this role.

Requirements

Master's degree in social work, psychology or a related human services field.
Licensure in the State of Oklahoma, in one of the mental health disciplines or under Board approved supervision for licensure.
Licensed LCSW, LPC, LMFT, LADC-MH, LBP, or under supervision required.
If licensed, employee must complete continuing education requirements required for licensure maintenance.
High degree of flexibility and efficiency.
Proficiency in MS Word processing software and intermediate general computer skills.
Good interpersonal skills and the ability to effectively communicate verbally with English-speaking clients.
Excellent written and oral communication skills.
Ability to move about facilities freely.
Casual professional dress is required with adherence to dress code requirements.
Access to reliable transportation.
Local travel is required.
Possess and provide verification of valid Oklahoma driver's license during employment with NorthCare.
Maintain and provide verification of current automobile liability insurance including uninsured motorist coverage.
Complete all NorthCare required training within 30 days of employment with NorthCare.
Must have TB test within 12 months prior to employment with NorthCare or obtain within 30 days of employment with NorthCare.

Responsibilities

Provides crisis management services, which may include face-to-face or telephone interviewing of the client; assessment of lethality to self or others; assessment of available support systems; and assistance in arranging for appropriate intervention, including hospitalization.
In the cases where a potential for harm exists, will complete appropriate assessment tools and, if needed and in consultation with management, will facilitate referral to appropriate level of care.
Consults with clinical leadership, as needed, to assist in risk management and clinical disruptions.
Coordinates intervention, outreach, and referrals to services to ensure continuity of care.
Any services that result in assisting clients in crisis must be documented in Avatar within 24 hours.
Follows up with consumers within 24 hours after the crisis to ensure safety. If no contact is made continues follow-up attempts daily for at least 5 business days.
Coordinates and completes follow up on all reported crisis services following best practices and within payor timeframes.
Provides integrated services that are welcoming, engaging, and accessible. These services address behavioral healthcare, physical healthcare, and social needs that are recovery focused, strengths based, culturally competent, client centered, trauma informed, and co-occurring capable.
Provides brief evidence-based counseling as needed to individuals or families.
Provides group therapy or other supportive services (e.g. case management, care coordination, wellness) if needed by the outpatient program.
Consults with other program staff within and outside of the agency to facilitate referrals.
Performs screenings, intake interviews, treatment plan reviews and other initial assessments, which includes the gathering of information about the client's symptoms, behavior, functioning, history, needs, goals and preferences.
Works as part of a multi-disciplinary team to support health behavior change in persons with behavioral health challenges.
Interact and collaborate in a genuine and professional manner with the client, their families, support system and community partners to assist with obtainment of treatment goals.
Obtains and maintain required credentialing including but not limited to licensure or under supervision status and Alcohol and Other Drugs Treatment Practitioner (AODTP) status.
Obtain and maintain understanding and competency in any other instruments required by agency payers' and/or commonly used by mental health/substance use professionals.
Participates in staff consultations for the purpose of case presentation and diagnostic and treatment planning for clients.
Participates in mandatory program meetings.
Supports the monitoring and tracking of outcome measures for clients.
Maintains clinical records in a timely manner, as required by agency policy, funding sources and accreditation authorities.
Complete all required forms in a manner consistent with requirements.
Maintains professional competency in the area under one's responsibility through appropriate readings, attendance at workshops and trainings.
Comply with all requirements to maintain professional certification(s).
