About The Position

The Crisis Services Floater provides daily assistance and supports to individuals in the Crisis Services Program who meet the criteria for access to crisis services due to critical need. This position utilizes applied behavior analysis techniques and positive behavior supports to provide consistent support and education to individuals in the program. Ensures program outcomes and objectives are being met, and ensures supports provided meet organizational guidelines, funder requirements and accreditation standards. This position provides flexible coverage to fill vacant shifts within assigned group of homes.

Requirements

  • Knowledge of disabilities, medical care and behavioral supports.
  • Must be able to support individuals with disabilities in a variety of situations.
  • Able to work with challenging behaviors and utilize appropriate de-escalation techniques.
  • Able to assess situations quickly and recognize the elements of a crisis state.
  • Able to implement behavioral plans with fidelity during high stress situations and able to project a calm and positive/neutral demeanor.
  • Models coping, problem-solving and social skills to encourage appropriate boundaries and attachments.
  • Maintains open and professional communication at all times including in high stress environments.
  • Communicates effectively with individuals, families/guardians, team members, medical providers and community members.
  • Able to work independently and provide supports in adherence to organizational policies and procedures, funder requirements and accreditation standards.
  • Knowledge of Microsoft Office suite and computer skills for utilizing a variety of computer systems.
  • Must have high school diploma or equivalent.
  • Must have at least 2 years of experience providing direct support to individuals with disabilities, including at least 1 year experience providing direct support to individuals requiring a high level of behavioral support.
  • This position requires driving personal or company vehicle.
  • Must have reliable vehicle and maintain current driver's license and proof of insurance.
  • Must complete and maintain all required trainings/certifications.
  • Must maintain confidential information in accordance with HIPAA regulations.

Responsibilities

  • Provides a high level of generalized positive behavioral interventions to individuals with complex behavioral support needs.
  • Supports individuals while their Individual Support Plan (ISP) and/or Behavior Support Plan (BSP) is being developed/revised, then provides interventions according to individualized plans once developed.
  • Reviews and implements activities related to ISPs and BSPs working towards plan outcomes and objectives.
  • Captures timely and accurate behavioral data while implementing BSPs.
  • Performs all required documentation related to agency and/or funder requirements and stays updated on any changes.
  • Reviews all organizational communications each shift.
  • Provides education, assistance, mentoring and support to individuals in the program while discovering their individualized needs.
  • Supports may include, bathing, personal hygiene activities, dressing, cooking according to dietary guidelines, maintaining a safe home environment, cleaning, laundry, and transportation to activities and appointments, etc.
  • Participates in updating the individual plan to reflect current situation and support needs.
  • Provides education to individuals regarding their human rights and ensures rights are respected.
  • Administers medications and medical care according to physicians' orders and communicates any changes in functioning, health status or behavior.
  • Provides education and support to individuals regarding individual-specific health issues, medications, and side effects.
  • Accompanies individuals to medical appointments, communicates physician's orders to team, performs RN delegations, and completes necessary documentation.
  • Provides assistance, education, and modeling behaviors for supporting community and social opportunities, and promotes communication/visits with friends, family members, other providers, etc.
  • Attends all required staff meetings and opportunities for ongoing education and training.
